Nominations are now being accepted for the Ontario Masters Fastball Hall of Fame.
Four people will be inducted into the Hall of Fame during a short ceremony at the 2014 OASA Masters Championship July 18-20 in Cambridge Ontario.  All four people will each receive a custom made individually personalized Hall of Fame ring.
Nominations will close on Monday April 14th. Nominations will NOT be accepted after this date.
The Ontario Masters Fastball Hall of Fame is not affiliated with the OASA or any other fastball organization/association other then the
Ontario Masters Fastball website.
 
Nominations
 
You can’t nominate yourself but anyone can nominate someone they feel is worthy, one nomination per person.
 
Eligibility
 
-    Players, coaches, managers can be nominated
-    Must be residents of Ontario
-    Can be active or retired
-    Must be at least 48 years of age in 2014
-    Must have participated in any 12 masters tournaments from the following list of sanctioned/recognized MASTERS tournaments.
-    OASA Masters, NAFA Masters, ISC Legends, Eastern Canadian Masters, North Bay World Senior Tournament, World Masters Games,
     Canadian Masters
-    Example : If a person participated in 10 OASA Masters Tournaments and 2 North Bay Tournaments they would be eligible or any       combination totalling 12 masters events.
 
We have an anonymous selection committee already in place who will vote from the list of nominees.
I will not be on the selection committee.
Some of the things our selection committee will be looking for are, longevity, individual and team accomplishments, character and sportsmanship.
 
Please include the following when submitting your nomination.......
 
1      Name of person you are nominating
2      Their age as of Dec 31st. of this year
3      Their current home town
4      Their home phone number
5      Their cell phone number
6      # of times they attended each tournament ( Example, 10 OASA, 6 North Bay, 1 NAFA, 1 WMG )
7      # of top 3 finishes at each tournament ( Example, 6 OASA, 3 North Bay )
8      Individual awards or recognition at all tournaments (MVP’s, top hitter, All World or All Star selections etc. )
9      Their ring size
10    Their uniform # that will appear on their ring
11    Their position that will appear on their ring
12    Their last name or nickname that will appear on their ring
13     A photo, preferably an action photo
14     Any additional comments on why you think this person should be considered for the Hall of Fame
         Please make sure you obtain permission from someone before you nominate them, you will probably have to
         work closely with whoever you nominate to make sure you get as much information as possible and to make sure the info is accurate.
 
Nominations will not be accepted unless all information has been supplied, all nominations will be kept confidential, only the the winners will be announced sometime after the nomination deadline.
